Abstract
The authors' experiences in implementing a team-based model of change for building and sustaining family-centered practices provide the background for this article. The focus is on challenges and strategies related to three components of the model: team participation, the decision-making process, and implementation of decisions made by teams as a result of workshop activities. The article includes a case example of one program's experience with the model
